The first of the two consents is the potential risks for surgery and explains that there are risks involved with this elective procedure such as postoperative infection or bleeding during or after the surgery, and short-term risks such as a thrombosis or phlebitis. Okay? So please review this and if you have any questions you can talk to Dr. Berger about that.

The second consent is an informed consent and a request for tubal reversal surgery. And this states that you do understand that this is a mini laparotomy or incision above your pubic bone and that you are aware there are risks of the surgery. We won't know if we can repair the tubes or not. There is no guarantee of that. And there is no guarantee that after we do repair the tubes that you can get pregnant. And let you know that is there is an alternative of IVF if you are interested in that instead of having tubal reversal surgery. And could you please review this?
